Kawasaki disease is an acute self-limited vasculitis of childhood that is characterized by fever, bilateral non-exudative conjunctivitis, erythema of the lips and oral mucosa, changes in the extremities, non-vesicular rash and unilateral cervical lymphadenopathy. Coronary artery aneurysms or ectasia develop in 15% to 25% of untreated children and may lead to sudden death or ischemic heart disease later in life, so early diagnosis and treatment with intravenous immunoglobulin and aspirin are important. As it is an uncommon disease and some patients lack sufficient clinical signs to fulfill the classic criteria, there could be problems in diagnosis. Here we present a Kawasaki disease one-year-old case with BCG vaccination reactivation, to point out that BCG reactivation might be a clue in the atypical Kawasaki case.
