Anti-e isoimmunization in twins: a case report

Maternal immunoglobulin (Ig)-G type blood group antibodies other than Rhesus
D (RhD) are an increasingly significant cause of alloimmune hemolytic disease
of the newborn (HDN). Anti-e-associated HDN has been reported rarely in
the literature, and is mostly responsible for mild anemia and hemolysis. We
describe herein the first case report of twin brothers with severe hemolytic
anemia and hyperbilirubinemia due to anti-e antibodies that required repeated
doses of intravenous immunoglobulin (IVIG) and erythrocyte transfusions. They
received a total of 4 g/kg IVIG therapy. One month after discharge they had
no hemolysis. In conclusion, isoimmunization due to subgroup incompatibility
such as anti-e should be considered in patients with hemolysis, anemia and
jaundice. Phototherapy, IVIG and transfusion are the treatment options.
