Clinical and electrophysiological evaluation of patients diagnosed with Wolff-Parkinson-White syndrome during the early childhood period

Wolff-Parkinson-White (WPW) syndrome presents with paroxysmal
supraventricular tachycardia and ECG findings of short PR interval with delta
wave. Sudden cardiac death can be the first symptom. Analysis of data of 45
patients aged ≤10 years who underwent electrophysiological evaluation for
WPW was performed. Thirty-three patients presented with tachycardia and
four with syncope; eight were asymptomatic. Atrial fibrillation was induced
in six patients, and it degenerated to ventricular fibrillation (VF) in two (1
of these patients was asymptomatic and the other had syncope); accessory
pathway effective refractory period was ≤180 ms in both. Ablation was
performed in 30 patients; the success rate was 90% (27/30) and recurrence
rate was 17% (5/30). Induction of VF in two patients suggests that the
prognosis of WPW in early childhood is not without risk. All patients with
WPW pattern should be assessed electrophysiologically and risk-stratified.
Ablation in patients with risk factors can prevent sudden death.
